In the ambulatory and hospital world, diagnosis is usually uncomplicated because of a clear cut clinical presentation. In nursing home patients, this is not the case. As an example, even without falls or trauma, the elderly patients with severe underlying osteoporosis can fracture hips just by turning in bed. This is also true in the spine and occasionally even in other long bones.
